We are registered as a private provider in the City of Clearwater. Please follow the below instructions.

Permitting Process...

1. Download the file(s) at the bottom of this page. (The Private Provider Job Site Form is only needed if we are doing the Plans Review)

2. Fill out the Notice to Building Official and notarize it. Fill out the Subcontractor Job Registration Form. Submit both with the permit request paperwork.

3. Upload the Certificate of Insurance, Qualifications Statement, ICI Resumes, and Duly Authorized Rep documents at the bottom of this page, any plans and affidavits with the permit request paperwork.

Plans Review...

If you are going to use us for Plans Reviews please fill out the Private Provider Job Site Form and submit it to the juisdiction. Then email your request to plans@ici.work
